Agency: Select the Agency that this request is for.
Effective Date: Select the date that this request needs to be processed.
Type of Request:
AC Name - First and last name of access coordinator
AC Email - Email address of access coordinator
AC Telephone Number - Telephone number access coordinator
End-User Name - First and last name of end-user
End-User Email - Email address of end-user
End-User Telephone Number - Telephone number of end-user
LoginID - 1st three characters are the agency abbreviation, the rest is up to the agency.
The maximum number of characters for a logon is 8; however, not all systems can handle 8 characters.
VITA recommends limiting a logon to 6 characters.
ACF2/UIDs: - a 24 character field containing the user identification string.
The UID provides a way to group users to minimize rule writing; the lst three characters are the agency abbreviation,
the rest is up to the agency.
Account Numbers - Identify the position account(s) to be used for billing system access.
Account number(s) must exist in VITA's billing table.

Agency Approver's Email: Input email addresses for your
Agency's Authorized Account Request Approver. Agency Authorized Account Request
Approver may consist of AITRs, ISOs, and Designees. When this form is submitted, those
individuals listed within these fields will receive an email from you via the COV
Account Request Website to approve or deny the request.
